Job Summary:
The Community Care Coordinator (CCC) is responsible for the professional and the highest quality delivery of health care services and shall provide leadership and supervision under the direction of the Health Services Director (HSD), having exceptional people skills for working with staff, residents, and families. All responsibilities will be conducted in a manner that is consistent with the Mission and Policies and Procedures of Chestnut Place.
Job Responsibilities:
General Duties
- Responds to all Resident emergencies. Provides direct care/medication administration to residents as needed, is visible and available to residents, families, and staff.
- Documents all pertinent health care services delivered including incident reports, medication error reports, and calls to physicians and families. This includes follow-up to any care given or planned. This documentation and follow-up must be completed within established timeframes.
- Maintains PCC health services software to include assessments to ensure that the services that care staff are providing to the residents are accurately reflected. Continuous updates of tasks/service plans to accurately reflect resident care needs/abilities/equipment, etc.
Training/Oversight
- Collaborates with HSD in training, supervising, and corrective action for Health Services staff.
- Assists with monitoring staff for delivery, oversight and documentation of all medications and treatments administered. Intervenes as necessary to ensure standards are upheld.
- Supports in-service efforts to comply with state regulations and provide ongoing training to staff.
Communication/Coordination of Care
- Participates in or directs change of shift reports. Communicates changes in health care status, care, and any pertinent information regarding the residents to the resident assistants (RAs) to ensure continuity of care for the residents.
- Assures that the Executive Director, Health Services Director, dietary, housekeeping, and activity departments are aware of the allergies, physical or psychological impediments and preferences of all residents as applicable.
- Is a primary liaison with family members to update or to explain care requirements/residents’ status.
- Acts as the first line nursing contact with physician offices and other service providers as required to coordinate appropriate care and services for residents.
- Reports any grievances to the Health Services Director and takes immediate action if necessary.
- Collaborates with established outside vendors and provides supervision of their work as applicable.
- Collaborates with HSD/scheduler to ensure that all staffing needs are being met for all shifts and covers shifts when needed; Enforces daily schedule and assignments for the staff.
Auditing/Compliance
- Monitors health services staff for compliance with policies and procedures, dress code, and teamwork.
- Participates in State surveys under the direction of HSD/ED; Assists with rollout, auditing, and enforcement of all Plans of Correction.
- Audits for compliance/accuracy in all aspects of healthcare delivery and education requirements. Proposes/engages in reasonable actions; utilizes Critical thinking/Root Cause Analysis concepts to devise plans to prevent recurrence. Assists HSD in identifying areas of improvement and establishing proactive measures to mitigate potential deficits.
- Helps monitor, track, and maintain nursing equipment.
- Audits for completion and ensures all admission assessments and care plan reviews are up to date. Ensures that updates are completed per State and/or Chestnut Place policies.
- Tracks all annual and scheduled Resident assessments; Ensures the process for Assessments, annual orders, PCP visits, and Care Plan meetings is carried out. Monitors Residents for significant changes (SC) and collaborates with HSD to ensure SC and accompanying assessments are completed as needed within established timeframes.
- Updates tracking systems as directed by HSD including but not limited to Line Lists, User Defined fields, Assessment schedules, etc.
- Understands and establishes services in compliance with the local, State and Federal regulations.
- Upholds all infection control practices. Stays abreast of changes and refers to CDC/state guidelines as needed to maintain appropriate IC measures.
Other Duties:
- Is available to participate in on-call scheduling and in filling shifts when necessary.
- Must assume additional responsibilities as desired by the HSD or Executive Director
- Performs all other duties as assigned.
Qualifications:
- Must be at least 21 years of age.
- Must have a valid RN/LPN license in the practicing state.
- Must demonstrate skills in clinical leadership and team building.
- Must be flexible and able to work every other weekend.
- Must be able to think, act, and intervene independently in both routine and emergency situations. Must be self-motivated and function as an “in charge” person.
- Must be able to relate to residents and colleagues in a confidential, courteous, and diplomatic manner under all circumstances.
- Must have previous experience with Alzheimer’s and dementia-related diseases.
- Must test negative for Tuberculosis.
Working Conditions:
A clean, ventilated, well-lit environment is provided by Chestnut Place. May be subject to the following hazards:
- Injury from tripping or falls.
- Injury from improper lifting techniques.
- Exposure to pathogens in bodily fluids, infections, odors, and behavior of residents.
- Exposure to chemical/cleaning solutions.
Demands of the Job:
- Must be able to move intermittently throughout the workday.
- Must be able to lift/push 60 pounds, carry, stand, squat, crawl, reach, and kneel using good body mechanics for sustained periods of time.
- Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fulfilled.
- Must be in good health and demonstrate emotional stability in order to cope with the physical, mental and emotional stress of this position.

Pay: $40.00 - $42.00 per hour
