We are seeking a compassionate and driven
Physical Therapist (PT) for a
critical-need full-time position at an SNF center in Williamstown, Massachusetts.
This is an excellent opportunity for a clinician who wants to make a meaningful difference in the lives of patients while working within a collaborative rehabilitation environment. You'll have the opportunity to provide hands-on care, develop individualized treatment plans, and help patients regain the skills and confidence needed to reach their functional goals.
The schedule offers
35 hours per week on weekdays with flexibility, providing room to create a schedule that works for both the clinician and the needs of the facility.

Responsibilities
- Complete physical therapy evaluations and assessments
- Develop individualized treatment plans based on patient needs and goals
- Provide skilled therapeutic interventions
- Help patients improve mobility, strength, balance, endurance, and functional independence
- Monitor patient response to treatment and adjust plans as needed
- Document evaluations, treatments, progress, and outcomes accurately
- Educate patients and caregivers regarding therapy and safe mobility
- Collaborate with nursing, therapy, physicians, and other members of the care team
- Promote patient safety and quality outcomes
Qualifications
- Doctor of Physical Therapy (DPT) or equivalent degree
- Current Massachusetts Physical Therapist license
- Strong communication and clinical skills
- Patient-centered approach to care
- Ability to work effectively in an SNF environment
- Adaptability and strong organizational skills
