Role: Primary Care Provider – Physician
Responsibilities
- Deliver comprehensive, patient-centered primary care to diverse and underserved populations.
- Provide acute, chronic, and preventive care, managing chronic diseases effectively.
- Coordinate care with behavioral health, dental, and specialty service teams.
- Utilize Electronic Health Records (EHRs) to document and manage patient information.
- Contribute to quality improvement and health equity initiatives.

Qualifications & Skills
- Strong clinical skills in primary care, including managing acute and chronic conditions.
- Experience working with diverse, underserved populations preferred.
