Primary Care Provider – Physician will deliver comprehensive, patient-centered primary care for diverse and underserved populations. In a collaborative, team-based setting, the role provides acute, chronic, and preventive care, including evaluation, diagnosis, and ongoing management of chronic diseases.
Key duties include coordinating care with behavioral health, dental, and specialty providers, using an EHR to document and manage clinical information, and contributing to quality improvement and health equity initiatives.
