Seeking compassionate
Registered Nurses (RNs) and
Licensed Practical Nurses (LPNs) to provide evidence-based, patient-centered care for post-acute, rehabilitation, and long-term residents in a skilled nursing setting.
Key duties
- Perform patient assessments and support care planning
- Administer medications and monitor patient response
- Provide wound care and other nursing interventions
- Document care and clinical findings in the electronic health record (EHR)
- Educate patients and families to promote understanding and adherence
- Collaborate with an interdisciplinary team to ensure coordinated care
- Participate in quality improvement and safety initiatives
Qualifications & Skills
- Current RN or LPN licensure
- Strong clinical assessment, medication, and wound care skills
- Proficiency with EHR documentation
- Effective communication and teamwork with patients, families, and interdisciplinary staff
- Commitment to compassionate, learning-focused, and patient-centered care
