Role: Responsible for delivering individualized support to individuals with intellectual disabilities or Autism Spectrum Disorder in community settings, under the guidance of the Program Coordinator.
Responsibilities
- Provide respectful, person-centered services that promote client choice and safety.
- Facilitate activities aligned with the individual’s interests and implement their treatment plan.
- Advocate for clients within the community and connect them to resources.
- Support health and wellness by monitoring well-being, administering medications, and attending medical appointments.
- Develop safety plans, support adherence to legal or court conditions, and ensure compliance with regulations.
- Maintain accurate documentation, data collection, and timely reporting.
- Collaborate with team members, share information professionally, and participate in care planning.
Qualifications
- High School diploma or equivalent; experience with individuals with disabilities preferred.
- Proficiency in Microsoft Outlook.
- Valid driver’s license and reliable transportation required.
