Role: Registered Nurse (RN) or Licensed Practical Nurse (LPN) providing compassionate, evidence-based, patient-centered care for post-acute, rehab, and long-term residents.
Key Duties
- Perform patient assessments and support ongoing care planning
- Administer medications safely and accurately
- Provide wound care and other skilled nursing interventions
- Document care in the electronic health record (EHR)
- Collaborate with an interdisciplinary care team
- Educate patients and families to support understanding and outcomes
- Participate in quality improvement and safety initiatives
Skills & Qualifications: Current RN or LPN licensure, strong clinical judgment, proficiency with EHR documentation, commitment to quality and safety, and effective communication and teamwork.
